I am searching to see if there are official forms needed for Florida to prepare an irrevocable trust for Medicaid eligibility purposes. I understand that there is a 5 year lookback period.

When we had to apply for Medicaid for my late husband, the administrator of the facility he was in was invaluable in helping me complete all the MANY forms. In addition, she directed me to a facilitator at our local hospital. Many of your questions can be answered by these folks. At least ask.

Florida document preparers are prohibited from doing anything related to Medicaid Planning. We are not even allowed to sell forms or kits. Only financial planners or attorneys may assist with medicaid planning. We forwarded the contact form you sent to one of our members who is a financial planner. I expect he'll contact you soon.
